Submit proper documentation to avoid denied claims

As a Delta Dental dentist, you’re required to submit documentation with claims for certain procedure codes. Claims submitted without the required documentation will be denied automatically.

To help make sure your claims are reviewed quickly and aren’t denied for missing documentation, please refer to the documentation requirements guide.

This helpful reference contains a full list of procedures that require documentation, including CDT codes, descriptions of the services and submission requirements. You’ll also find pointers for submitting x-rays, chart notes and photos.
